Getting Ready For Your First Appointment
When you come to our office, please remember to bring the following:
- A list of all medication you are taking or their original containers
- Valid ID such as your driver’s license
- Health insurance card
To make your office visit go more smoothly, you may complete new patient forms in advance. Please select and print the attached forms, fill them out completely and bring them with you to your appointment.
We accept most PPO insurances:
- Medicare
- Anthem Blue Cross / Blue Shield
- United Healthcare
- Aetna
- Cigna
- CHAMPVA
- APWU
- Health Net
- Tricare
- Meritain Health
- AARP (Secondary)
We do not accept any HMO plans.
